Just type in your Last Name, First Name and take the quiz. Submit when you are finished. Once you have opened it, you must complete it, there is NO going back at a later date once it has been opened. So, allow yourself time, about an hour or so, before you open the quiz. If you make an error, or your printer broke, or your Internet went down, etc., then you will have to stay after school one day this week to complete the quiz in class. No retakes will be allowed.
